No frills is definitely a consistent theme in reviews and I think that is the most accurate description.

Starfish is consistantly the best seafood and fish in the area. Besides the great food the location can’t be beat. It is a bit rustic but the views and intercoastal actvity make the meal complete.

Came here on another recommendation from the locals and had one of the best lunches. All we ordered was freshly caught. Right next to the fishing boats. It's worth coming here. Anyway, it'll be back. You can't go wrong with everything you order. The darkened group sandwich was amazing.
